SURVEY FOR PARENTS

Please complete a short survey on behalf of RMIT University that will help our club.

Dear Parent,

South Melbourne FC has been selected by the Royal Melbourne Institute of Technology (RMIT) as one of nine clubs to assist in a research study they are undertaking on food choices that families make around community sport. This research is supported by VicHealth.

Our club will receive a small payment for each completed survey, so your support is greatly appreciated. These funds will be re-invested in supporting our junior football programs.

We kindly ask your help in completing the survey as soon as possible. One parent in each family that has a child/children playing at our club should respond. It is open until June 2nd.

You can do the survey on any device you have, and it is relatively short and simple to complete. It takes approximately 8-10 minutes to complete.

The survey is completely anonymous and asks how families make food choices around training nights and game days.

Only one parent per family should do the survey.
